PCN: Weoley and Rubery
Mission statement
We aim to provide the highest standard of acute care, chronic disease management and to screen for medically and socially important diseases while encouraging positive health beliefs. We strive to be accessible and friendly to the population we serve, while fostering the personal motivations of the practice team.
Premises
We practice from recently extended and refurbished buildings. Funding was provided by a grant for university teaching and via the National Health Service notional rent scheme. Ownership of the building is shared amongst the partners.
Patients’ services
We run a comprehensive appointments system, offering ten minute slots. In addition we offer same day slots for urgent appointments. The system was developed by the whole practice team with the intention of maximising availability of both acute and chronic services and minimizing stress for the team. Disease management sessions are available to patients with asthma, diabetes, hypertension, and chronic obstructive pulmonary disease. We also offer child health and minor surgery services. We offer advice on female health, teenage sexual health, birth control, menopause. Counsellors and alcohol workers also hold.
